The best reason to have an epidural steroid injection is not for the relief of back pain, but rather for the relief of leg pain caused by spinal nerve compression. Often called sciatica , leg pain caused by nerve irritation is the best reason to have an epidural. These symptoms can be caused by disc herniations or spinal stenosis . We know very well, that general low back pain symptoms are not well treated with an epidural steroid injection, and this should be avoided as a treatment for this type of pain. Back pain is not a good reason to have an epidural injection.

The benefits from the first shot only lasted 2 weeks. The second and third set of injections lasted about 90 days. In November, I was ready to have surgery. My EMG and nerve conduction tests proved that the nerves were "sleeping" before I was. I have gotten an MRI now and he prescribed a physical Therapist that is really helping me. This is the third set, the other two were Prescribed by another doctor that I did not get the hands on treatment from. I can not say enough good things about this group of doctors, PAs and their group.

By using a high frequency ultrasound probe the wrist is imaged by placing the probe transverse across dorsiflexed wrist. The median nerve and ulnar artery is identified. At the level of the distal wrist crease, the needle is passed into the skin superficial to the ulnar artery, penetrating the flexor retinaculum. The needle is advanced toward the median nerve. The steroid solution is injected just under the flexor retinaculum then retracted and redirected deeper to the ulnar side of the median nerve. This allows the median nerve to be completely surrounded with the steroid solution. [ 3 ]

For many people, back pain goes away on its own or with nonsurgical treatments. Epidural steroid injections shouldn't typically be used as a first-line therapy for back pain relief, but that doesn't mean they can't play a role in treating pain. But injections won't cure the underlying cause of back pain, and they provide only temporary relief. Unfortunately, in many cases, chronic back pain can't be cured, but must instead be managed, like other chronic conditions—and patients must have realistic expectations of what epidurals can do.
