Use propofol with caution in patients with cardiac disease, heart failure, intravascular volume depletion (., hypovolemia, dehydration), abnormally low vascular tone (., sepsis), or hemodynamic instability (., heart failure). Patients with these conditions may be more susceptible to propofol-induced hypotension. A lower induction dose and slower maintenance rate should be used in such patients and any patient with severe systemic disease (ASA-PS III or IV). Consider the risk of hemodynamic effects on the cardiovascular system in patients who are severely overweight (obesity), as the propofol dosage will be higher. Compensate for cardiac, circulatory, and pulmonary disease or insufficiency and correct fluid deficits prior to propofol administration. If fluid therapy is contraindicated, other measures (., lower extremity elevation, vasopressor initiation) may be used to offset hypotension. Adjust infusion rates slowly and avoid rapid boluses in order to minimize hypotension. If hypotension occurs, it may be responsive to drug discontinuation, IV fluid administration, and/or vasopressor therapy.

When trying to prevent low blood sugars, it helps to understand their most common causes. The table below outlines the most triggers for lows. If your are currently experiencing either frequent or severe reactions, look carefully at this list to see what might be causing them. Once you identify a cause, decide upon a correction. Remember: when low blood sugars are frequent or severe, it is almost always necessary to reduce current insulin doses. If lows blood sugars happen infrequently, try to identify the trigger so you can make adjustments when you encounter that situation again.

Children and adolescents <16 years taking high doses of fluticasone propionate (typically ≥1000 micrograms/day) may be at particular risk of systemic effects. Systemic effects may occur, particularly at high doses prescribed for long periods. Possible systemic effects include Cushing's syndrome, Cushingoid features, adrenal suppression, acute adrenal crisis and growth retardation in children and adolescents and more rarely, a range of psychological or behavioural effects including psychomotor hyperactivity, sleep disorders, anxiety, depression or aggression. Consideration should be given to referring the child or adolescent to a paediatric respiratory specialist.

Candidiasis of the mouth and throat (thrush) occurs in some patients, the incidence increasing with doses greater than 400 micrograms of beclometasone dipropionate per day. Patients with high blood levels of Candida precipitins , indicating a previous infection, are most likely to develop this complication. Patients may find it helpful to rinse their mouth thoroughly with water after using the inhaler. Symptomatic candidiasis can be treated with topical anti-fungal therapy whilst still continuing with Easyhaler® Beclometasone 200 micrograms/dose treatment.
